A recent inquest into the death of a woman who was undergoing heart surgery has found that there were gross failings in providing blood to her during the procedure. A spelling mistake had caused a delay to the blood supply and the surgeon was not aware that blood was not available until he was performing the operation. 

In the cases that we see, the cause of our client's injuries can be something as seemingly insignificant as a spelling mistake, but such errors can have very significant consequences for the people involved and their families.
